As a member, you have access to a range of services designed to support your overall health, including:
- Annual Physicals
- Sports Physicals
- Sick Visits
- Phone and Virtual Visits
- Chronic Care Management
- Follow-up Appointments
- Lab Work Discussion and Optimization
- Preventative Health Screenings (e.g., cancer, heart disease, diabetes)
- Wellness Coaching (nutrition, fitness, and stress management)
- Prescription Refills and Management
- Specialist Referrals
~ Registration fee: $149 (One-time)
~ Individual Plan: $169 per month
~ Couple Plan: $239 per month
Includes 2 people, which can be any combination of adults and children under 25 years old.
~ Family Plan: $279 per month
Families include 2 parents and up to 2 unmarried children under the age of 25.
~ Additional children: $20 per month, each, when added to the family membership.
